At SLM Services, LLC, we partner with leading insurance carriers to provide accurate and complete long-term care assessments. Our mission is to ensure that individuals receive the appropriate level of care through precise and thorough evaluations. We are seeking a skilled and detail-oriented Registered Nurse to join our team on a PRN/as-needed basis to conduct these vital assessments with patients in their homes or assisted living facilities in Austin Texas and Surrounding cities

Key Responsibilities

  • Conduct comprehensive assessments of clients' physical, mental, and emotional health.
  • Ability to assess an individual's level of ADL and IADL dependency by observing them mimicking their activities of daily living, Ability to communicate with patients by asking various questions regarding their current health conditions, complete an cognitive assessment, Monitor blood pressure, body weight and height
  • Complete thorough assessment reports for insurance carriers, ensuring accuracy and completeness.
  • Communicate effectively with clients, families, facility staff
  • Compassion and Willingness to Listen and Document Details on Patients health diagnosis or changes to diagnosis, medications, etc

Qualifications:

  • Current and unrestricted Registered Nurse (RN) license in Texas.
  • OIG/SAM/Criminal Background/Nursys Verification.
  • Minimum of 2 years of experience in longterm care, case management, or a related field.
  • Strong assessment and critical thinking skills.
  • Excellent written and verbal communication skills.
  • Ability to work independently and manage time efficiently.
  • Own a personal laptop/tablet/ desktop computer with printer and scanner (required)
  • Must have a valid driver license and car insurance
  • Flexibility to work on an asneeded basis, including potential travel to client locations in Austin Texas or surrounding cities.

Pay:
$ $50.00 per hour
