Responsibilities

Coordination of care with entire care team
Completing home care nursing visits
Educating patients and family members
Preparing timely, complete and accurate clinical notes
Communicating with healthcare providers and other care team members

Qualifications
-
LVN or LPN in the State of Texas:

Home Health experience (1 year at least):

Knowledge of Home Health regulations and general healthcare compliance:

(Preferred but not required) Experience with HCHB EMR:
Truly cares for people and will go above and beyond to provide outstanding patient care

Has a passion for nursing and teaching
Has outstanding communication skills (written, verbal and nonverbal)
