- Collaborate closely with interdisciplinary teams to ensure comprehensive care plans tailored to each patient's needs.
- Maintain accurate documentation of assessments.
- Active Registered Nurse (RN) license in the state of Georgia.
- Minimum of 2 years of nursing experience, preferably in home health or a related field.
