Description

DUTIES OF POSITION

Maintains the quality of consumer services in accordance with the Agency's objectives and policies, through planning, coordination, and implementation and evaluation of the home services.


POSITION RESPONSIBILITES
Plans, organizes, directs and evaluates operations to ensure adequate and appropriate care and services are provided.
Complies with applicable laws and regulations.
Coordinates and schedules the daily workload of each staff member/contractor thereby utilizing Agency personnel to their fullest capacity.
Provides daily supervision of caregiver employees to ensure the quality of services and maintenance of standards.
Ensures the accurate and timely communication of scheduling changes between office and field staff.

Reviews and investigates the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of consumers.

Directs staff toward the appropriate use of community resources.
Appraises staff performance during their probationary period and annually,

Ensures new hire orientation and skills checklist as well as annual training on competencies and regulatory trainings are completed timely and with passing scores where appropriate.

Provides one-on-one teaching and counseling necessary to enhance the delivery of services and the development of home services staff.

Identifies and evaluates consumer needs and appropriateness of home services by reviewing the initial intake screening form before assignment of home services staff.

Develop the Service Plan after assessing the consumer and consulting the consumer and/or family member/responsible person.
Review and update Service Plans per policy.
Participates in community related functions as necessary.
Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures.
Sees consumers in the home setting as needed.
Participates in consumer record reviews.
Reviews each schedule for discharges and direct care staff supervisory visits.
Reviews weekly schedule submitted for changes or updates.
Ensures that all pertinent paperwork is in the consumer record and is complete.
Ensured the Agency's continuing education program i.e. in-service education and continuing training.
Performs other duties as requested by the Agency Manager.


JOB CONDITIONS
Position is stressful in terms of meeting deadlines.
Primarily a desk job which involves mínimal and occasional lifting of supplies and charts.

On occasion, one may be required to bend, stoop, reach and move consumer weight up to 250 pounds lift and/or carry up to 30 pounds.

Travel is required to and from consumer's home when supervision of services is scheduled.
The ability to communicate well, both verbally and in writing in English is required.
The ability to access consumers' homes which may not be routinely wheelchair accessible is required.

Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the consumer's condition and to perform consumer care/services.


EQUIPMENT OPERATION
Use of multi
- line telephone, calculator and other office equipment and supplies.


COMPANY INFORMATION

Has access to all consumer records and consumer financial accounts which may be discussed with the Agency Manager, the Executive Director, the Director of Finance, and the Quality Assurance Department.

Has access to department's staff personnel records.


QUALIFICATIONS

  • Must be a high school graduate.
  • Must have at least one-year experience training/experience in the provision of Home Care Services.
  • At least one year of supervisory/managerial experience.
  • Must have a criminal background check and other checks as required by PA regulation
  • Must have current CPR certification and First Aid training.
  • Must have a valid driver's license.
