Job Description
The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.
The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning.
The Inpatient Social Worker must be highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.
Knowledge and experience with evidence-based practices, including trauma informed care framework. Knowledge and experience working with co-occurring populations (chronic health conditions, mental health and substance use)P&BH:
Reports to LEAD LICENSED MENTAL HEALTH PROFESSIONAL
Responsibilities
Works closely with psychiatrists, case management and treatment team to have an understanding of the patient's utilization plan, appropriateness of continued hospitalization, observation status, length of stay and quality issues-inpatient only
Ability to identify cases and prioritize day proactively and participates in daily huddles and treatment team to assess needs and coordinate services
Completes initial and ongoing assessment of the patient and family's response to diagnosis, living environment, functional status, financial situation and discharge planning needs
Assesses the capacity of the home caregiver to cope with the patient's needs in the development of the plan of care including transportation needs and emergency financial assistance-inpatient only
Conducts a comprehensive patient/family assessment to ensure appropriate referrals, identify barriers, clarify or determine realistic goals for the treatment/discharge plan in the acute care setting-inpatient only
Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner
Supports the milieu as assigned by the psychiatrist, nurse manager and charge nurse
May provide clinical supervision to non-licensed and student interns-both
Conducts individual, group and family therapy and interventions-both
May perform other duties as assigned or requested and job specification can be modified or updated at any time
Qualifications
Required Education:
Resident of Counseling or Social Work within six (6) months of licensure exam.- Inpatient only Masters Degree in Counseling, Masters Degree in Social Work (MSW), or Masters Degree in a related field from accredited university
Preferred Education:
Required Experience:
If in recovery, two (2) years of sobriety required
Preferred Experience:
Inpatient Acute Psychiatric experience
Inpatient only:
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) with two (2) years of experience in mental health or substance abuse field
Preferred Certifications and Licensures:
Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) or PhD with two (2) years of experience in mental health or substance use treatment
