DEFINITION

Provides assessments, evaluations, referrals, case management, and educational instruction for clients in partnership with local EMS Agencies.

ESSENTIAL AND MARGINAL FUNCTION STATEMENTS

Essential and other important responsibilities and duties may include, but are not limited to, the following:

Essential Functions:

1. Assesses clients' needs and provides referrals to community providers and social services agencies.
2. Collects and provides client information in accordance with established state and federal requirements for confidentiality; and maintains client files regularly.
3. Maintains a variety of automated and manual records and files; provides operational information and statistical data for management use.
4. Duties are varied in nature and require independent judgment to determine the proper course of action within the limits of policy and procedure.
5. Identifies social, economic and physical needs of clients.
6. Assesses client's support systems, available community resources and other factors to plan, develop, and implement an appropriate plan.
7. Utilizes agency guidelines, state, and federal regulations to determine/confirm eligibility for programs and services.
8. Refers clients to appropriate community medical, emotional, economic and social support organizations; advocates for or assists the client in obtaining such services.
9. Prepares complete and accurate case notes.
10. Writes correspondence, reports and other written materials.
11. Uses standard office equipment, including a computer, in the course of work.
