As a Client Care Coordinator, you will play a vital role in providing tailored care solutions and support to our clients and their families.
Your responsibilities will include conducting care consultations, developing personalized service plans, coordinating care arrangements, and ensuring ongoing client satisfaction through quality assurance visits.
Key Responsibilities:
- Conduct care consultations with potential clients and families to understand their needs and preferences.
- Develop personalized service plans based on client assessments and discussions.
- Coordinate and schedule care services, including client introductions to Care Professionals.
- Conduct quality assurance visits to ensure service excellence and client satisfaction.
- Maintain regular communication with clients, families, and internal teams to address concerns and modify service plans as needed.
- Adhere to company policies, procedures, and ethical standards in all interactions and activities.
Requirements:
- Minimum of an associate degree or equivalent education preferred.
- At least one year of experience in home care, health care, or a related field.
- Availability to work Monday to Friday, with occasional weekends as required.
- Clear background check (criminal, motor vehicle record) and valid driver's license.
- Strong interpersonal skills with the ability to communicate effectively and empathetically.
- Excellent organizational abilities and attention to detail.
- Ability to work independently while maintaining confidentiality and meeting deadlines.
