Key Responsibilities:
Outreach and Education:
- Conduct outreach activities to identify individuals due for Medicaid renewals.
- Educate community members about the importance of Medicaid coverage and the renewal process.
- Provide information on eligibility criteria, documentation requirements, and deadlines.
- Must have a valid driver's license and access to a reliable vehicle for transportation to and from Health Resource Administration (HRA) offices and other community locations.
Application Assistance:
Follow-up and Support:
- Follow up with applicants to remind them of upcoming deadlines and missing documentation.
- Provide ongoing support and guidance to individuals throughout the renewal process.
- Liaise with Medicaid offices and other relevant agencies to resolve any issues or concerns raised by applicants.
Documentation and Reporting:
- Maintain accurate records of all interactions with applicants, including documentation of assistance provided and outcomes.
- Collaboration and Teamwork:
- Collaborate with other members of the healthcare team, including case managers, social workers, and healthcare providers, to ensure coordinated care for individuals.
- Participate in team meetings, trainings, and other relevant activities to enhance knowledge and skills.
Qualifications:
- Bachelor's degree in a relevant field such as public health, social work, or healthcare administration preferred.
- Bilingual in Bengali and English or Chinese (Mandarin and Cantonese) and English
- Previous experience working in healthcare, social services, or community outreach preferred.
- Knowledge of Medicaid eligibility criteria, renewal processes, and documentation requirements.
- Strong communication and interpersonal skills, with the ability to interact effectively with individuals from diverse backgrounds.
- Excellent organizational skills and attention to detail.
- Ability to work independently and as part of a team.
