Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults.

We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems.

As a part of our continued success and growth, we are seeking qualified applicants for a
Outpatient Nurse (LPN) for our Medication Management Clinic.


Under the direction of a Physician, Nurse Practitioner, Physician Assistant, or the Clinic Manager, the LPN provides routine clinical and administrative support for the team, and basic medical attention to assigned patients.


Key Job Duties and Responsibilities:


  • Preps patient charts which include utilizing Michigan automated Prescription System (MAPS) database.
  • Processes perscription refill requests.
  • Escort patients to rooms, interviews patients, measures vital signs, including weight, blood pressure, pulse, and document all information in patient's chart
  • Distributes screening forms to patients as appropriate.
  • Assists with toxicology screens.
  • Completes a medication review with the patient, and assists in medication education.
  • Administers ADHD testing as required.
  • Conducts Suicide Risk Asssements as appropriate.
  • Give instructions to patients as instructed by physician or NP/PA
  • Monitors status of laboratory and other diagnostic tests ordered. Ensure all related reports, labs and information is filed and available in patients' medical records
  • Takes telephone messages and makes sure that information is routed appropriately to Psychiatrist or Nurse Practitioner.
  • Resolves telephone inquiries including communication with pharmacies and insurance companies as well as taking escalated patient calls
  • Gather samples per provider orders, log them out appropriately and present them to the provider for review before distributing the samples to the patient.
  • Assists in escorting patients to hospital when needed
  • Assists with completion of Medication Prior Authorizations

Hours:
This position is Monday-Friday, day shift. No weekends required

Qualifications:

  • Graduation from an accredited school of practical nursing.
  • Current LPN Licensure in the state of Michigan.
  • Basic understanding of psychiatric nursing theory, and practice, with previous psychiatric clinical experience preferred. Previous work experience in providing support to psychiatric patients in an inpatient or outpatient setting highly preferred.
  • CPR/HWC certification required or obtained within the first week of employment.
  • Knowledge of pharmaceuticals and demonstrated knowledge and competency related to Sample Medication Policies and Procedures.
