- Reviews applications and medical reports to become familiar with applicants.
- Does initial history of participant's health status, develop, implement and periodically updates individual care plan for each participant in collaboration with the clinical manager.
- Responsible for case management of primary participants including but not limited to:
- Arrange and monitor medical appointments and transportation to appointments. Post all information on calendar.
- Coordination with PCP office and contacting pharmacy for medication refills.
- Coordination of PT, OT and speech and mental health therapy as indicated
- Coordination of participant services with other community agencies.
- Provides healthcare teaching to participants and their families.
- Assists participants with ADL's.
- Obtaining and recording of vital signs
- Assists with distribution of food and supervises participants during meals.
- Administers prescribed medications and treatments at the program; supervises self-administration
- Responds to reporting and documentation requirements of all regulatory agencies including but not limited:
- General physical assessments monthly and as needed, assessing the client's basic health status, records and related health data;
- Documents unusual findings and healthcare teaching in participant's record;
- Documents daily flow sheets, medications and treatment sheets, monthly nursing notes and quarterly report to MD;
- Follow up to assure timely return of MD quarterly report;
- Signs and dates all documentation as appropriate;
- Administers flu shots and immunizations, as indicated.
- Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided.
- Follows established hospital infection control and safety procedures.
- Assisting as necessary in the delivery of other ADH Program services; including and not limited to leading Therapeutic Activity programs
- Participate in multi-disciplinary staff meetings.
- All other duties are required.
- Graduate of an approved school of practical nursing.
- Current license in the Commonwealth of Massachusetts to practice as a Licensed Practical Nurse.
- BLS certified
- Previous experience working with patients in an acute care setting preferred.
- Requires knowledge of processes and community resources and agencies relative to health, protective services, crisis intervention, disability, etc.
- Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families, and to supervise graduate students.
- Must be comfortable in a fast paced and dynamic work environment (sense of humor is valued).
- Excellent English language skills to summarize and communicate orally and in writing moderately complex information in varied formats to internal and external customers.
- Cultural sensitivity and comfort with a wide range of racial and ethnic populations. Bicultural/Bilingual competency a plus.
- Computer literacy with ability to use standard word processing, spreadsheet programs and e-mail (i.e. MS Word, Excel, Outlook) and web browsers.
- Must be able to maintain strict confidentiality of all personal/health sensitive information
