We're currently seeking a part-time Community Life Coordinator to serve residents in our independent and personal care units at St. John's Herr Estate, a Luthercare community in Columbia, PA. The position is Part Time Day-shift, Monday through Friday.

WHAT YOU DO:


  • Adheres to all Luthercare standards, applicable state/federal regulations, and follows general safety protocols
  • Receives resident's complaints and requests for assistance. Visit residents to inspect/evaluate units regularly to ensure compliance appropriate level of care.
  • Acts as campus mediator in resident disputes. Services as management representative on Residents' Council. Advises residents, committees, and assists with program resources.
  • Participates in interdisciplinary team resident care planning and other interdisciplinary committees and prepares social work notes and recommendations to be included in residents' medical records.
  • Assists with resident transfers between various levels of care, ensuring financial updates are received as needed for level of care transfers.
  • Monitors hospitalization and transfers of residents with family members or appropriate hospital personnel for any change in placement status.
  • Responds to fire alarms and medical emergency pendants and calls and manages any incident reports submitted.
  • Participates in support groups for the terminally ill or grieving residents/families.
  • Coordinates and oversees campus Resident Review Committee.
  • Supports campus events, such as open houses and communitywide events, as part of the campus leadership team.

QUALIFICATIONS/REQUIREMENTS:


  • Bachelor of Science (B.S) in social work or equivalent combination of education and experience.
  • Three years of directly related experience in social work
  • Demonstrated knowledge of the psychology of the elderly.
  • Demonstrated knowledge of the common physical and mental illnesses and their associated symptoms.
  • Two years' experience in the administration of independent living units and experience providing social services in a healthcare facility is preferred.
