  • Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing nonprofit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ microcommunities scattered across Orange County, Los Angeles County and the Inland Empire._
The pay range for this position is
$ $24.00 per hour, depending on experience.


Responsibilities

  • The Outreach Lead Care Manager will use a variety of communication modalities, communityresources, rhetorically appropriate approaches, and triageoriented screening methods to performing outreach & engagement to prospective members across
social determinants of health and in culturally and linguistically competent ways.

  • An Outreach Lead Care Manager will have an expert understanding of how to utilize different communication modalities when trying to reach their assigned Member list.
  • The Outreach Lead Care Manager will be accomplished in the form of timeutilization and will field high amounts of communications and enrollment conversion rates.
  • Outreach Lead Care Manager will be able to use rhetoric effectively when speaking to Members, affiliate parties, care team members, PCP's, etc.
  • Outreach Lead Care Manager will have familiarity with techniques such as traumainformed care, motivational interviewing, cultural and linguistic competency, and deescalation.
  • Ability to enroll 5% of assigned caseload.

Documentation:

  • Outreach Lead Care Manager will be responsible for appropriately navigating and documenting all engagement frequency and content in Illumination Foundation's EMR and other documentation platforms as required by contracting entities, in accordance with standards of practice as required by Illumination Foundation and associative contracting entities

These practices include:

  • Documenting level of engagement such as frequency and duration of encounters within 24 hours.
  • Documenting evaluative Member details which factor into decisions made to refer clients to alternative resources.
  • Documenting client disengagement as well as engagement.
  • Completing the social determinants of health screening tool for each client, so as to triage them into the most appropriate caseload environment.
  • Proficiency in doubledocumenting in EHR, external platforms (such as Molina CCA and Blue Shield Care Connect), and the assignment roster.
  • Documenting on any other such desirable reporting metrics as outlined and required by contracting entities.

Preferred Experience/Minimum Qualifications

Required:

  • High School Diploma or equivalent.
  • 2 years relevant experience.
  • Valid CA Driver's License and eligibility for company vehicle insurance.

Preferred:

  • Bilingual in English and Spanish.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams)
  • Minimum of 1 year of sales and/or marketing experience.
  • Some knowledge of Medi-Cal Managed Care Plan delivery systems
