Cardea Health is an Oakland based nonprofit healthcare organization founded to connect marginalized populations to the clinical and supportive services they need to improve their health, remain stably housed in their community, and age in place.

Cardea Health's programs fill a critical - and growing - gap in the system of care. Among people experiencing homelessness, a growing number are living with complex medical needs. They face poor quality of life, increased morbidity and mortality, and medical and social destabilization.

But they have few housing options:

they need a higher level of care than what is offered through permanent supportive housing, but they do not wish to live in an institutional setting and have limited access to hospice or palliative care.


Cardea Health specifically targets this vulnerable population, providing a range of clinical services within permanent supportive housing sites to both address their medical needs and allow them to live independently in their communities, addressing a critical need within the system of care.


About this role:


Cardea Health is initiating clinical care and services at the Kelly Cullen Community in the Tenderloin neighborhood of San Francisco.

The Kelly Cullen Community is in the Tenderloin neighborhood and is home to 174 individuals with extremely low incomes, including those that experienced homelessness and are currently living with health challenges.

On-site social services are run by the Tenderloin Neighborhood Development Corporation, clinical services are provided by Cardea Health.

Medically frail residents are provided nursing care, caregiver support and medical casework up to and including end of life care.


Cardea Health is search of an organized and motivated person to work as a care coordinator alongside a diverse clinical team at the KCC Program.

The care coordinator will provide a variety of functions to support Cardea Health clinical programs at the KCC.

The care coordinator supports the day-to-day operations of a clinical team that cares for individuals who have experienced chronic homelessness.

Care coordinators also assist KCC residents in navigating their healthcare network.

Cardea Health staff collaborate with housing programs and other social services to create a safe, independent living environment for a dynamic and vulnerable population.


Required Qualifications:


  • High School Diploma or equivalent.
  • Experience as a medical assistant or administrative assistant is relevant.
  • Must have personal, reliable transportation to and from shifts
  • Must be 18 years of age or older
  • Selfdirected and able to work with mínimal supervision
  • Must be detail oriented and highly organized.
  • Must comply with all agency policies
  • Must be COVID vaccinated or willing to be vaccinated.
  • Prior experience with homeless or incarcerated populations is desirable.
  • Proficiency in Microsoft Outlook and Microsoft Word is required.

Essential Duties and Responsibilities:

The care coordinator will assist clinical staff and KCC residents in the coordination of medical and behavioral health care


Duties include:

  • Maintain a calendar of upcoming resident medical and behavioral health appointments. Communicate upcoming appointments to clients, clinical staff, and transportation services.
  • Assist clinical staff and residents in the scheduling of client medical and behavioral health appointments.
  • Communicate with local pharmacies regarding KCC resident prescriptions. At the direction of nursing staff request medication refills, deliveries, and transfers.
  • Request relevant medical records of Homekey residents from hospitals and community clinics. Transmit medical records and other authorized requested documentation to partner clinics to facilitate care coordination.
  • Interface with KCC resident healthcare coverage plans to avoid interruption in services and facilitate care.
  • Take telephone messages and provide feedback and answers to patient/physician/pharmacy calls.
  • Ensure adherence to infection control standards on site among staff and residents.
  • Answer calls to nursing station and respond independently to when appropriate.
  • Assist nursing staff with light clerical duties including filing and making copies.
  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.
  • Other duties and responsibilities as required.

Physical Demands:
