Case Manager - Sullivan County

JOB DESCRIPTION:


Under the general supervision of the Case Management Coordinator, the Case Manager (CM) provides "Hope Through Hospitality" and will support the needs of individuals who are experiencing homelessness residing in various locations approved by the Sullivan County Department of Family Services (DFS), providing provisions of basic needs, and creating a safe, secure, and confidential residential living environment.

The CM provides non-judgmental compassion and care by listening to client's concerns and providing non-directive counseling.

The CM acts as a role model, guiding clients and facilitating appropriate behaviors such as daily living skills, self-care, personal interactions, social relationships, constructive time management, following program policies and procedures.

Must be flexible with schedule to meet the needs of the clients which will include some evenings and weekends. Additional hours may be required to meet program regulations, contract obligations and client needs.


QUALIFICATIONS FOR THE POSITION:

  • Bachelor's Degree in Social work, Psychology, or related field.
  • Experience working in Human Service Field
  • Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people including individuals recently incarcerated, recent crime victims, etc.
  • Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for HONOR's mission.
  • Must have and maintain a valid New York State driver's license.
  • Detail-oriented and have competent literacy and writing, documentation, communications, and interpersonal abilities (friendly, courteous, helpful, ability to work as part of a team).
  • Bilingual preferred but not required.
  • SOAR Certified within 90 days of employment.

JOB DUTIES AND RESPONSIBILITIES:

  • Receives and completes required DFS documentation for new placements.
  • Assigns room to new placements and provides toiletries, room keys, etc.
  • Completes documents for check-in, check-out, law enforcement contacts, etc. and provides notification to DFS as they occur.
  • Completes a Comprehensive Assessment and Independent Living Plan within 24 hours of admission with the Resident to include referrals, appointments, and goals. Reviews ILP with Residents weekly, biweekly and/or monthly and revises/updates as required by OTDA Regulations.
  • Makes and/or coordinates referrals for medical, dental, eye care, mental health provider, Health Homes, substance use treatment, etc. Conducts follow ups to these referrals to ensure attendance and adherence to requirements.
  • Provides written and oral reminders to residents about upcoming appointments.
  • Completes daily room checks monitoring room inventory, cleanliness, maintenance, etc.
  • Completes daily room checks to confirm the client is staying in the room. Notifies DFS immediately if it has been determined that a client has abandoned the room.
  • Maintains current, timely (within 72 hours) and accurate documentation of services provided to clients in Electronic Health Record including but not limited to weekly progress notes and contacts with community-based providers. Files all paperwork in client charts within 72 hours.

Part Time Position Available:


Saturday/Sunday 9am - 5pm

Tuesday-Friday 5pm -10pm
