    Are you looking for a different approach to helping others navigate the complex health care system? We're looking for a compassionate Case Manager who is ready to make a difference in the lives of low-income adults living in the Mission.

    The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services.

    We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve with a focus on the Latino Spanish-Speaking Communities.

    The Mission Neighborhood Resource Center (MNRC) was founded in 2002 as a community response to the neighborhood's growing homeless population, the Mission Neighborhood Resource Center advocates for housing and resources to enhance the physical, social, emotional, and economic health of homeless residents in the 16th street corridor and surrounding Mission neighborhood. MNRC employs a service model based on harm-reduction principles and practices that focus on community building, basic human needs, and integrated health services for a diverse population as they move toward greater stability and well-being.

    We are currently looking for qualified candidates for our Case Manager (Bilingual) role. Please note: this role requires an applicant to be bilingual (English/Spanish). This position reports to the Program Director of the Mission Neighborhood Resource Center and is a Regular, Full Time, Non-Exempt role with a starting hourly wage of $29.00 to $32.00with full benefits .

    Primary Objective:

    In conjunction with a multidisciplinary treatment team, the Case Manager will provide on-going case management and crisis counseling to Resource Center participants. The Case Manager will provide housing, immigration and benefits support as well as assist clients to connect with MNRC's treatment team and San Francisco medical and behavioral health system programs as needed.

    Essential Functions/Responsibilities:

    1. Perform individual comprehensive assessment of Resource Center clients' psychosocial history, mental health status, medical needs, housing, food and other needs for basic support. Determine eligibility and necessity for case management, medical, and psychotherapy and psychiatric treatment, if appropriate. With clients' assistance, develop care plan delineating steps necessary to accomplish activities, goals and objectives.
    2. Implement the plan through the provision of interventions, coordination with integrated services team and/or through facilitated referrals.
    3. Utilizing a participant-centered, harm reduction model to provide supportive services to individuals, families and groups of patients.
    4. Utilizes motivational interviewing, cognitive behavioral and health psychology treatment methods. Utilizes depression screening tool (PHQ-9) and other evidence based screening tools as appropriate. Follows the SAMHSA SBIRT (Screening, brief intervention, referral and treatment) model for patients with substance use problems.
    5. Refer clients to appropriate shelter case management beds (Senior, Jazzie's Place). Assist clients to find, apply, access and stabilize in housing.
    6. Assist clients in compiling documentation and filling out forms for immigration and public benefits' eligibility, including County Adult Assistance Programs, Social Security disability programs, and Medi-Cal, among others.
    7. In conjunction with Drop In Services Manager, assign lockers to clients, and provide locker rules and locker contract. Receives and documents monthly locker fees according to protocol. Assist clients to connect with MNRC treatment team and to access the San Francisco medical and behavioral health system programs as needed.
    8. Conduct home visits and attend appointments with clients. Create a hospitable environment and provide services to clients with the utmost dignity and respect.

    Requirements

    1. Bachelor's Degree in Social Work, Psychology, Counseling or a related field, and at least two years paid work experience with homeless adults with multiple disabilities. Masters' Degree in Social Work (MSW) and/or Masters in Family and Child Therapist (MFT) degree preferred. Past experience working with homeless adults and dually and triple-diagnosed individuals desired.
    2. Bilingual Spanish/English required.
    3. Knowledge of motivational interviewing, cognitive behavioral and health psychology.
    4. Must be willing to provide support, advocacy and develop relationships following harm reduction principles with a very diverse homeless population, including Spanish speaking immigrants, day laborers, women, LGBT, African Americans, active drugs users and people with mental illness and other disabilities.
    5. Ability to work with de-escalation techniques, conflict resolution and emergency response
    6. Excellent interpersonal communication and writing skills.
    7. Ability to work professionally and ethically in a multi-cultural team setting, including ability to maintain a high level of confidentiality.
    8. Ability to work a flexible schedule, including evenings.
