THIS IS A BILINGUAL POSITION. TO BE CONSIDERED FOR THIS POSITION, APPLICANTS MUST BE FLUENT IN BOTH ENGLISH AND SPANISH.

DEFINITION:


Under general supervision, provides professional social counseling, support, and clinical case management to various populations within Hamilton County in programs related to prevention, rehabilitation and development of self sufficiency.


Provides care coordination to a caseload of medium to high risk families that have been referred to the CHANT model for services.

Minimum Qualifications:


Bachelor's Degree in Social Services, Human Services, Behavioral Sciences, Nursing, or Health Sciences with at least one (1) year of work experience in assignment area field; A valid driver's license is required.


ADDITIONAL REQUIREMENTS:

Some positions require additional training or certifications depending on assignment.


PREFERRED QUALIFICATIONS:

Home visitation experience preferred. Bilingual language skills in English and Spanish are preferred.


PHYSICAL REQUIREMENTS:


Work requires working with high risk, confrontational, mentally and physically ill, and economically disadvantaged populations with exposure to infectious diseases.

Work may involve working after regular business hours in high crime areas. Extensive home visiting is required.
