Overview:
The RN Transfer Center, under the supervision of the Director and/or Clinical Manager, is responsible for facilitating admissions/transfers to and within NAH.
S/he seeks assistance and support when necessary, assists in data collection, and facilitates admissions or transfer of patients for patient care.
Maintains accountability for own practice and those functions s/he delegates. Within a specified work period, plans and coordinates patient placement and physician consults.Responsibilities:
Administrative
- Updates Daily Bed Poll in accordance with Emergency Management Systems.
- Assists with ensuring the physician on call roster is kept uptodate.
Patient Care
- Obtains pertinent medical history and information from referring physician.
- Prioritizes and problem solves to meet changes in patient needs with regard to admission/transfer/resource availability.
- Makes appropriate specialist referrals.
- Collaborates with Transfer Center physician to ensure the level of care and resources are available for patient acceptance.
- Communicates frequently with referring facilities regarding updates, changes, transport, etc.
- Coordinates wait and return and outpatient visits.
Data/Reporting
- Completes a thorough and timely record of all admissions, transfers, and physician referral line calls.
- Reports hospital deaths to the medical examiner, per FMC policy.
- Assists in data collection and reporting as needed.
- Responsible for sending out STEMI and Stroke alerts in a timely manner.
Bed Placement
- Collaborates with the Administrative Coordinator in the placement or transfer of patients.
- Makes appropriate admission status assignments, taking into account diagnostic tests, labs, etc.
- Serves as a resource for Teletracking troubleshooting.
Compliance/Safety
- If required for the position, ensures all certifications and/or licenses are uptodate and valid prior to expiration dates.
- Completes all company mandatory modules and required jobspecific training in the specified time frame.
- Responsible for reporting any safetyrelated incident in a timely fashion through the Midas/RDE tool; attends all safetyrelated training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
- Adheres to regulatory and organizational standards, to include policies and procedures.
Qualifications:
Education
Graduate of accredited School of Nursing

- Preferred
Certification & Licensures
Active, unrestricted, Arizona RN License or valid, participating compact license
- Required

Experience
Three years acute care experience
