Posting Summary

  • Rutgers, The State University of New Jersey, is seeking a Patient Navigator for NJMS-MedicineInfectious Disease. This position provides clients with medical case management services as defined by the Health Resource Services Administration (HRSA).
Among the key duties of this position are the following:

  • Provides client-centered services that link clients with health care, psychosocial, and other services.
  • Coordinates and followsup to ensure that medical treatments have been provided.
  • Ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client's and other key family members' needs and personal support systems.
  • Ensures that comprehensive medical case management is provided including the provision of treatment adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
  • Key activities include (1) initial assessment of service needs; (2) development of a comprehensive, individualized service plan; (3) provision of services required to implement the plan; (4)monitoring to assess the efficacy of the plan; and (5) periodic reevaluation and adaptation of the plan as necessary over the life of the client.

    Minimum Education and Experience
  • Bachelor's Degree in a Health, Public Health, Social Work or other relevant field required; plus two (2) years' experience in the development and/or operations of community programs required.
  • Equivalent education, experience, and/or training may be substituted for the degree requirements.

Certifications/Licenses

Required Knowledge, Skills, and Abilities

  • Computer literacy with competence in Microsoft Office components required.
  • Must possess excellent written and verbal communication skills; strong organizational skills; detail oriented
  • Able to function independently and work flexible hours; friendly, outgoing, sociable; openminded & nonjudgmental; caring, empathic, compassionate; committed, dedicated, persistent; creative & resourceful.

Preferred Qualifications

  • Knowledge of Newark community preferred. Bilingual skills (English/Spanish)
  • Phlebotomy certification
  • NJDHSS HIV counselor certification

Physical Demands and Work Environment

  • PHYSICAL DEMANDS:
Computer literacy, typing, working at multiple workstations in a shared environment, ability to speak clearly, write proficiently. Extensive reading and extended sitting, ability to move from unit to unit. Position requires standing and walking over 2/3 of the time.

Talking, hearing & sight are essential; requires clear close & distance vision, peripheral & color vision, depth perception and ability to adjust focus.

Position requires lifting to 10 lbs; on occasion more based upon patient demands
