We are seeking a dedicated Patient Care Coordinator to join our team and provide exceptional care coordination and management services.

Typical Tasks include:

Collaborating with clients, families, and healthcare professionals to develop customized care plans.
Conducting assessments to determine individual client needs and preferences.
Coordinating and scheduling caregiving services based on client requirements.
Supervising and training caregivers to ensure quality care delivery.
Regularly performing client visits to monitor care quality and address any concerns.
Maintaining accurate and uptodate client records and documentation.
Acting as a liaison between clients, families, and healthcare providers to facilitate effective communication and optimal care management.
Experience in home health care, geriatric care, or longterm care (highly desirable).
A genuine passion for helping others and providing highquality care.
Empathy and patience in dealing with clients and their families.
A proven ability to lead and manage a team of caregivers and support staff.
Proficiency in using care management software and technology.
Willingness to work nontraditional hours as needed.
