- Provide coaching, modeling, and support to empower individuals and families, helping them find strength and resources.
- Respect the integrity and confidentiality of individuals receiving services.
- Foster the development and strengthening of natural supports within the community.
- Actively participate in treatment with child, youth, and family teams, aiming to keep them in the home and avoid out-of-home placements.
- Empower individuals and families through modeling, advocacy, coaching, and support.
- Develop and maintain positive relationships with community partners, serving as a liaison for NorthCare.
- Manage complex cases, coordinating services through the wraparound process.
- Maintain accurate records and documentation in compliance with requirements.
- Meet patient contact/caseload standards as established by the Director.
- Ensure prompt communication via phone, text, and email.
- Maintain consistent work attendance and promptly communicate any late arrivals or absences.
- Participate in meetings, consultations, and training sessions to expand your knowledge.
- Provide services in the community, including clients' homes, local venues, or via telehealth.
- Transport patients to various locations in the community and provide services in the home.

- High School diploma or equivalent.
- Lived experience as a caregiver navigating mental health services or similar systems of care.
- Obtain ODMHSAS Family Support Provider certification within 60 days of hire.
- Obtain ODMHSAS Wellness Coach Certification within 60 days of hire.
- Minimum of one (1) year experience required in a community mental health or mental health-related field.
- Proficiency in MS Word and intermediate computer skills.
- Strong interpersonal and communication skills.
- Access to reliable transportation for local travel.
- Valid Oklahoma driver's license and automobile liability insurance.
- Obtain CPR, First Aid, and Therapeutic Options certifications within 30 days of employment.
- Complete all NorthCare required training within 30 days of employment.
