Full-Time Onsite Position

CARE MANAGER RN


Our Award Winning Client, an esteemed healthcare provider operating nationally, is seeking a dedicated Care Manager RN to join their dynamic team.

Case Management is a collaborative practice involving patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners, and the community.

The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management and utilization management, while recognizing the patient's right to self-determination, the significance of the social determinants of health, and the complexities of care.


Required Qualifications:


  • Bachelor's degree in nursing or equivalent education/experience.
  • Master's degree in nursing or a healthcarerelated field.
  • RNs with 20 years of RN experience will not be required to obtain a BSN.
  • Any previously agreedupon effective date for obtaining a bachelor's degree in nursing between individual and facility will remain in effect.
  • Any collective bargaining contract with specific time frames for obtaining a bachelor's degree in nursing will supersede the above point and will remain in effect.
  • Exceptions will be considered on a casebycase basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader.
  • RNs employed prior to 1/1/17 are encouraged to obtain a bachelor's degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment.
  • Upon hire: California Registered Nurse License.
  • 2 years of experience in a healthcarerelated field (Acute, Ambulatory, Post-Acute, etc.).

Preferred Qualifications:


  • Upon hire: CCM or ACM certification.
