Under the general supervision of the CFS Program Manager; the Advocate Training Coordinator will follow the Independent Living philosophy to provide a variety of tailored, individualized supports to meet the changing needs and preferences of CFS clients by coordinating various opportunities for consumers by utilizing the Advocate Training Coordinator for success.

These Services include working with the families and communicating in the preferred language while being respectful to the consumer and family's culture.

Additional assistance is provided by directly and/or indirectly providing resources and training for locating and obtaining generic resources in housing, benefits, transportation, personal care attendants, medical services, employment or education, nursing facility transition, as well as other individualized need areas.


JOB SPECIFICATIONS

Required:


  • Associate (AA) Degree in Human Services field or Three years direct
hands-on experience working with individuals with a variety of

disabilities while coordinating/navigating services.

  • Ability to communicate effectively both orally and in writing.
  • Professional English writing ability, including usage, grammar, spelling, punctuation and vocabulary.
  • Ability to keep and file records.
  • Willingness to accept work direction from others.
  • Ability to make effective presentations.
  • Proficient use of basic office equipment, including Microsoft Office Suite.
  • Immediate access to a vehicle, and proof of current driver's license and car insurance.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Complete intake on consumers, assess their needs according to CFS Action Plan. Establish goals and time-guidelines to assure effective and efficient delivery of services. Monitor progress of consumers towards stated goals.
  • Coordinate Services closely with consumer and family (Resource Development, Housing, Benefits, Assistive Technology, etc.) to assure a high quality and comprehensive CFS Action Plan is developed to meet the needs of the consumer.
  • Develop a comprehensive training curriculum that includes a variety of areas leading to development of a CFS plan of support and/or training.
  • Provide a copy of the CFS Plan to the consumer, and provide follow up opportunities to assess ongoing needs, progress toward goals, and needed resources.
  • Advocate for and assist consumers in obtaining benefits such as Social Security Disability Insurance, Supplemental Security Income, Med-i-Cal, Medicare, Food Stamps and other county, state and federal benefits.
  • Work with the community resources to locate appropriate, affordable and accessible housing for the consumer.
  • Advocate for consumers concerning all CFS services, and provide information and referral, as well as follow-up on an ongoing basis.
  • Provide peer support and peer-counseling.
  • Develop and maintain good working relationships with community service leaders.
  • Be familiar with legal requirements of local, state and federal programs, and other related providers to be able to provide assistance, guidance and advocacy to both consumers and family members.
  • Track all referrals and or leads provided to consumers for follow up.
  • Coordinate training and assistance on topics identified in the Individualized CFS Action Plan.
  • Compile and report quarterly performance statistics monthly.
  • Handle and organize high volume of paperwork.
  • Represent the organization to educate the broader community about people with disabilities and available services through the organization.
  • Work with bilingual consumers, as needed.
  • Assist and work with the Consumer and the Family in the preparation of consumer's cases for Appeal Hearings.
  • Maintain consumer files in accordance with SCRS Policies and Procedures, Data Base and vendor compliance standards.
  • Work with generic resources staff members to ensure that consumers receive all services needed.
  • Refer consumer to generic resource programs that are related to Peer Support and System's Advocacy.
  • Work with community agencies to provide consumers with additional needed services, which SCRS does not provide.
  • Participate in a variety of training opportunities to continuously improve knowledge and skills as it relates to CFS.
  • Represent the organization at outreach and other events.
  • All other duties as assigned.
