JOB SUMMARY:
Responsible for assisting the care team (provider, medical assistant,

behavioral health provider, etc.) by coordinating services for patients who are part of the assigned panel, especially those with serious, complex, chronic or psychosocial issues.

Clinic Lead


JOB DUTIES:
Responsible in facilitating access to appropriate health services by assisting with the coordination of referrals, admissions, discharge and/or transfer of patients to specialty care, hospitals, nursing homes, rehabilitation facilities, or residential care facilities, including the following:

1. Provide an effective communication link between patient, medical staff, behavioral health staff, rehabilitation facilities, hospitals, etc.

2. Schedule, coordinate and track referrals for diagnostic testing and specialty care.

3. Assist in coordination of care with other providers in the community, ensuring that information goes when and where it is needed.

4. Facilitate provider communication at regular intervals throughout patient's hospitalization or stay at other facilities.

5. Maintain ongoing communication with discharge planners, case managers, and care coordinators at facilities to which patients are periodically admitted.

6. Ensure all patients are tracked and data entered into systems for follow up and reporting.

7. Coordinate with medical staff to ensure that case management services are provided to patients with complex medical and/or psychosocial problems.

8. Handle protected health information in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

  • The Referral Coordinator will have access to protected health information during the course of his/her work activities. The Referral Coordinator will use this information to communicate medically necessary information to other health care providers to facilitate care for the patient.

9. Consistently demonstrate and uphold CCCHC's principle of providing quality health and human services to the medically underserved and low-income populations in a culturally sensitive manner.

  • Knowledgeable of the health care system including managed care contracting, Medi-Cal, Medicare, and local IPAs.

Education and Experience Requirements

  • Bilingual in English/Spanish preferred (or English/Vietnamese if assigned to Garden Grove).
  • High School diploma or GED required.
  • Two years of college desirable.
  • Prior case management experience preferred.
  • Prior experience working with underserved populations preferred.

Normal clinic environment with:


  • Physical requirements for this position include: hearing, seeing, speaking, feeling, reaching and repetitive motions. Walking and/or standing is routinely required.
  • This position requires sitting for long periods of time, including work at a computer for extended periods on a daily basis.
  • Ability to exert up to 40 pounds of force in order to lift, carry, and pull or move objects.
  • Ability to handle multiple tasks while also being tactful, diplomatic, and empathetic.
  • Ability to organize and schedule work effectively.
  • Ability to communicate effectively and tactfully with patients, customers, managers and other levels of personnel, in person and on the telephone.
  • Ability to pay close attention to detail.
  • Ability to maintain composure under conditions of urgency and/or time constraint.
  • Ability to work under pressure in a fastpaced environment.
  • Ability to handle and maintain patient confidentiality.
