We are currently looking for a motivated RN Case Manager to care for patients in the East Valley. If you believe in providing exceptional patient care, operating with professionalism and integrity, and you want to be a part of an excelling company; we would love to hear from you

Some of the work perks of this position is that most patients reside in one building and we're looking for someone with a charismatic, outgoing personality to tender the relationship with the building

Utilizing their depth of experience and knowledge, hospice nurses are responsible for:

  • Completing hospice evaluations as requested (admission if comfortable and training would be provided)
  • Conducting in home visits as assigned; assessing the patient's condition, and teaching the patient and family
  • Developing care plans based on diagnosis and needs
  • Administering medications and treatments as prescribed by physician
  • Communicating patient needs and changes with the team as necessary

Requirements:


  • Valid AZ RN license (multistate compact license is acceptable as well) (_required_)
  • Minimum of two years of Hospice nursing experience (_required_)
  • Valid AZ Driver's License and automobile insurance (_required_) with reliable transportation (_required_)
  • Pass background check (_required_)

Pay:
From $85,000.00 per year
