Position:
Case Manager


Description:

Case Manager assists in the development of client objectives, reports on client progress, prepares clear and concise reports and interacts with caregivers, counselors and others.


Duties and Responsibilities

  • Oversee assessment of clients
  • Assist in development of client objectives
  • Ensure client health and safety issues are addressed at job site
  • Set up and monitor client files
  • Participate in client service planning meetings
  • Write reports on client progress
  • Meet with clients to discuss progress
  • Communicate effectively with support teams
  • Coordinate with consultant services, if any
  • May be required to provide direct service to consumers
  • May be required to assume Program Director duties in some circumstances
  • Other duties as assigned by Program Director or Regional Director

NEW HIRE BONUS - $750 hiring bonus after 60th day of full attendance (must work full shift to be counted towards the 60 days).


Requirements:
Must pass a criminal background check and job-related physical. Must possess a California driver's license, own vehicle with liability insurance and a clean driving record.
