Position:
Case Manager

Description:

Case Manager assists in the development of client objectives, reports on client progress, prepares clear and concise reports and interacts with caregivers, counselors and others.

Duties and Responsibilities

Oversee assessment of clients
Assist in development of client objectives
Ensure client health and safety issues are addressed at job site
Set up and monitor client files
Participate in client service planning meetings
Write reports on client progress
Meet with clients to discuss progress
Communicate effectively with support teams
Coordinate with consultant services, if any
May be required to provide direct service to consumers
May be required to assume Program Director duties in some circumstances
Other duties as assigned by Program Director or Regional Director

NEW HIRE BONUS - $750 hiring bonus after 60th day of full attendance (must work full shift to be counted towards the 60 days).

Requirements:
Must pass a criminal background check and job-related physical. Must possess a California driver's license, own vehicle with liability insurance and a clean driving record.
