    Job Description

    The Targeted Case Manager's primary duty is to provide case management services to adult patients. The Targeted Case Manager will be responsible to assist the individual with obtaining needed access to housing, medical, social, educational and other identified services.

    Qualifications

    • Bachelor's degree in a behavioral science (including psychology, sociology, social work, family studies, human services, counseling, or another approved human service degree). A minimum of one (1) year of full-time employment working directly adults in a human service setting; in the field of Substance Use;
    • Successful completion of Targeted Case Management training approved by the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (KBHDID) and completion of recertification requirements approved by KBHDID every three years
    • Ability to work in a fast paced, dynamic environment.
    • Valid Kentucky driver's license.

    Responsibilities

    • To be well versed on local programs available, and to be proficient in knowing as much as possible on all other Kentucky programs.
    • Assisting the client in gaining access to needed medical, social, educational, or other needed services and supports, including:
    • · Responsible for conducting a comprehensive assessment and a periodic reassessment of an individual's strengths and needs; and
    • ·Responsible for assisting an individual to gain access to identified medical, social, educational, and other service needs.
    • · Provide case management services for a maximum caseload of 25 Individuals
    • Represent agency programs, policies and procedures accurately in the community and with other social service agencies.
    • Ensure that all documentation complies with KY Medicaid and accreditation standards.
    • Participate in bi-monthly supervision with TCM billing Supervisor.
    • Participate in monthly staff meetings and lead in trainings/discussions as assigned.
    • Maintain certification requirements for TCM pursuant Kentucky Board regulations.

    Job Type: Full-time

    Expected hours: 32-40 per week
