Description

• Provide compassionate and comprehensive care to patients in a hospice setting
• Assess and monitor patients' physical and emotional wellbeing
• Administer medications and treatments as prescribed by the healthcare team
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Educate patients and their families about the disease process, treatment options, and endoflife care
• Assist with pain management and symptom control
• Coordinate and communicate with other healthcare professionals involved in patient care
• Provide emotional support to patients and their families during difficult times
• Maintain accurate and uptodate patient records within a 24hour time frame

• Valid RN license in [TX]
• Minimum of [2] years of experience as a registered nurse in hospice or palliative care
• The ability to properly assess and identify the need for hospice admissions
• Proficient in hospice care and have a strong knowledge base of hospice needs, and the services provided to patients and their families. Must possess a strong understanding of state and federal compliance and regulations.
• Excellent communication and interpersonal skills.
• Compassionate and empathetic nature
• Ability to work effectively as part of a multidisciplinary team
• Strong organizational skills and attention to detail
• Proficient with the Kinnser EHR/EMR system
