Create personalized nursing care plans to ensure comprehensive care
Conduct ongoing assessments to monitor and address evolving health needs
Implement recommendations from healthcare providers seamlessly

Maintain accurate and up-to-date health documentation

To qualify, you'll need:

Graduation from an accredited nursing program
Active NY State RN license
Valid driver's license
Experience in mental health, disabilities, or community nursing preferred
