1.

PURPOSE:

This scope of work is for the development of new and existing functionalities required to help support the
Florida Department of Health's (Department) Health Management System (HMS). Contractor will provide
these services to the Department.

2.

TERM:

This scope of work will begin on 7/1/2022 or the date on with the purchase order is issued, whichever is
later. It will end at midnight, Eastern Time on 6/30/2023. The State of Florida's performance and obligation
to pay under this purchase order and any subsequent renewal is contingent upon annual appropriation by
the Legislature and satisfactory performance of the Contractor.

4.

CRIMINAL BACKGROUND SCREENING:
The Department will conduct a criminal history record check, including fingerprinting, on the consultant
ssigned by the Contractor prior to the consultant commencing work for the Department. The Department
retains sole discretion as to whether the results of the criminal history record check will result in the
Contractor's employee being disqualified from performing services pursuant to this agreement.

5.

CONTRACTOR QUALIFICATIONS AND EXPERIENCE:
Contractor staff assigned to this agreement must possess the following minimum qualifications and

experience:
years demonstrated experience with implementing Health Care Systems.
years demonstrated experience with the Department's HMS system, other clinic
management, or electronic health record software.
years demonstrated experience writing, developing user and system requirements
year demonstrated experience with Agile software development, project management
nd testing practices.
years demonstrated experience in analysis, testing and flowcharting and unified modeling.
years demonstrated experience using SQL.
years demonstrated experience with formal software testing.
years demonstrated experience with root cause analysis and troubleshooting.

6.

CONTRACTOR RESPONSIBILITIES:

6.1.

SERVICE TASKS:
Contractor will perform the following tasks in the time and

manner specified:
Develop user requirement documents, design documents and screen presentations based on
user requirements, user input and feedback as directed by the Department.
Provide research and analytical services for defects, enhancements and systems as they emerge,
s required as directed by the Department.
Meet with the program offices to collect user requirements and facilitate the development of
Requirements documentation for defects, and enhancements to the HMS system as directed by
the Department.
Facilitate development using object-oriented design methodology and design using the Unified
Modeling Language and or Flow Charts as directed by the Department.
Develop test cases and scripts to test the application and provide support, software testing, and
quality assurance to ensure that the application meets requirements, per the user requirements
s directed by the Department.
Provide work estimates, communication and updates for Release Planning, Sprint Planning and
Daily Scrum, daily in the time indicated on the work tickets assigned by the Department, to
implement system feature enhancements or additions and bug fixes.
Document and follow Office of IT processes and ensure compliance with Department Standards
s directed by the Department.
Complete job required training as assigned by the Department. Completion verification may
include certificates, sign-in sheets and/or verification via the Department's Learning
Management System.

DATA SECURITY AND CONFIDENTIALITY TASK:
The Contractor, its employees, subcontractors, and agents must comply at all times with all
Department data security procedures and policies in the performance of this scope of work as
specified in the Data Security and Confidentiality document attached to the purchase order.

6.2.

DELIVERABLES:
Contractor will complete and submit the following deliverables to the Department in the time and

manner specified:

Monthly:
Provision of System Analyst services in the time and manner specified in Tasks

7.

METHOD OF PAYMENT:

7.1. A purchase order will be issued to the Contractor.
7.2. The method of payment for this purchase order is unit rate.
7.3. The Contractor will not receive payment in advance for goods or services described in this scope of
work.
7.4. The Contractor must submit an invoice monthly that provides a detailed accounting of the deliverables
performed during the invoice period for which payment is being requested.
7.5. The Contractor is responsible for the performance of all tasks and deliverables contained in this scope
of work.

8.

PERFORMANCE MEASURES AND FINANCIAL CONSEQUENCES:
ll deliverables and related tasks must be completed 100% as specified. Failure to satisfactorily complete or
submit a deliverable in the time and manner specified will result in a financial consequence as indicated

below:
8.1. Failure to complete and submit Tasks in the time and manner specified will result in a
payment reduction equal to 12 percent of the total monthly invoiced amount.
8.2. Failure to comply with Task 6.1.9, DATA SECURITY AND CONFIDENTIALITY will result in a payment
reduction equal to 10 percent of the total monthly invoiced amount for each week the Contractor is not
in compliance.

9.

CONTRACTOR TRAVEL REIMBURSEMENT:
The Contractor will not be reimbursed for any travel expenses under this agreement.
