Medical Case Manager

Position Summary:


The Medical Case Manager's primary function is to facilitate and ensure that clients receive the most appropriate level and type of care needed as per the Ryan White program standards of care and agency policies under the direction of the Medical Case Manager Supervisor.


Responsibilities:


  • Provide all HIV clients who accept case management with a comprehensive assessment of basic physical, medical and psychosocial needs and access to HIV ancillary services
  • Evaluate and document assigned patients for program eligibility according to program criteria
  • Develop a patientcentered care plan at least every 6 months and/or as any major changes occur
  • Complete and follow up on referrals as needed by clients
  • Support patients with scheduling clinical and nonclinical appointments and lab work
  • Maintain a complete patient medical case management record that includes intake information, care plan and case management services notes based on client's acuity
  • Enter required information in CareWare within the allowed timeframe
  • Provide culturally and linguistically appropriate information, education and referral to target individuals or HIV positive individuals that are out of care
  • Assist in developing promotional strategies that will increase awareness of outreach and education services within our scope of care
  • Advocate for clients and act as a liaison between medical providers and community resources
  • Create and maintain area resources and referral networks
  • Attend required training per program requirements
  • Perform other duties as assigned

Qualifications:


  • Bachelor degree preferred and 1year experience in HIV care and prevention or Associate's degree in social service from an accredited program and 2 years of experience in HIV care and prevention
  • Culturally competent and experienced with marginalized communities / LQBTQIA+ and communities of color
  • Bilingual/Spanish speaking a plus
  • Works well with a diverse population in a nonjudgmental and supportive manner
  • Have strong interpersonal skills
  • Valid, unrestricted driver's license, reliable automobile and current automobile insurance required by CT state law

Pay:
$ $25.00 per hour
