- Explains hospice services and benefits to patients and families
- Submits necessary documentation to facilities after patient visits
- Articulates the regulatory framework for the Spiritual Counselor role
- Participates in the Interdisciplinary Team (IDT)
- Provides spiritual care support and coordinates community clergy involvement as requested
- Identifies the need for additional IDT interventions
- Communicates patient and family needs effectively to the IDT
- Collaborates with the IDT for coordinated spiritual care support
- Maintains updated care plans reflecting current issues and interventions
- Respects family beliefs and environment
- Provides counseling aligned with patient and family beliefs
- Supports patients and families through death, grief, and bereavement
- Acts as a liaison with the clerical community
- Performs assigned duties
- High school diploma or GED required
- Bachelor's degree in Divinity, Social Sciences, or related field preferred
- At least one (1) unit of CPE preferred
- Minimum of one (1) year experience in spiritual/pastoral support required
- Minimum of one (1) year experience in pastoral support related to loss, grief, or trauma preferred
- Mathematical Skills: Ability to work with numbers and calculations
- Language Skills: Strong written and verbal communication skills
- Other Skills and Abilities: Proficiency in English, understanding of hospice philosophy, high integrity, counseling skills, compassion, and empathy
- Criteria for qualification
- Documentation required for clergy-related degree or training
- Ordination or commission documentation required
- Requirements for theological degree or equivalent training
- Educational requirements in counseling or related field
- Additional training provided by employer within 90 days of hire
- Specific educational qualifications needed
