    Description

    Job Title: Care Transition Pharmacy Technician

    Company: Tampa General Hospital

    Under the direct supervision of a registered pharmacist, the Care Transition Pharmacy Technician provides technical support to ensure safe and effective transitions in care at admission and discharge. The technician collaborates with the patient care team, updating and documenting accurate medication lists in electronic health records. Responsibilities include medication records review, patient profile review, and staff development. The technician must be registered with the Florida Board of Pharmacy, hold a high school diploma or GED, have four years of pharmacy experience, and possess PTCB technician certification.

    Qualifications:

    • Registration with the Florida Board of Pharmacy
    • High School diploma or General Equivalency Diploma (GED)
    • Four years of pharmacy experience, preferably in a community setting
    • PTCB technician certification
    • Completion of a current/active Pharmacy Intern permit may be substituted if a State of Florida Registration Pharmacy Technician permit is not held
