- Responds to referrals to identify patient's skin, wound, and ostomy needs
- Conducts advanced assessments of patient's skin, acute and chronic wounds, stomas and fistulas
- Identify modifiable and non-modifiable risk factors so preventative measures can be appropriately implemented.
- Graduate from an accredited school of nursing. Completion of BSN or higher nursing degree by 2025
- Current NH Licensure as a registered nurse.
- Minimum of three years as a Wound Ostomy Clinician or specialty certification in wound or wound/ostomy care.
- Must possess good communication skills to work with clients, community services, and the health care team.
- Ability to achieve the following competencies: Basic Life Support, Care of the complex ostomate, Management of the complex wound, Vacuum Assisted Closure (VAC) Therapy, Compression Therapy, Mentoring, Must complete a minimum of six hours of continuing education in the management of the skin, wounds and ostomies.
