Registered Nurse – Case Management- Case Manager - Travel - (CM RN)
StartDate: 3/16/2026
Available Shifts: 8 D
Pay Rate: $ $
Transitions Program Department
Orientation will be approx 2 weeks in office at Kaiser Martinez 200 Muir Rd MTZ 94553.
After orientation the job will be remote--Applicant must have in home Wifi. A laptop and phone will be provided.
Candidates must reside within 60 miles of the facility for the duration of the assignment.
Transitions is a short term, telephonic, case management program. Patients are managed x 30 days with a minimum of weekly phone calls. The caseloads are patients. We are open 7 days a week from 8:00a to 4:30p. We have a weekend rotation every 7 weeks (both Sat and Sun). The entire team works remotely.
Required Qualifications
Registered Nurse, Case Manager
Case Management Experience required
Outpatient or Inpatient Experience requireed
Discharge Planning Experience required
Patient Care Coordination experience
required
BLS
Preferred Qualifications
Utilization Review Experience
Epic/Health Connect experience
Medicare and MediCal benfit knowledge
