    Shift: Days / 8a-4:30p, 8x5, 40 hrs per week
    Length: 13 weeks

    Requirements:

    • CA License
    • 2 years experience
    • Case Management Experience required
    • Outpatient or Inpatient Experience required
    • Discharge Planning Experience required
    • Patient Care Coordination experiencerequired
    • BLS
    • Utilization Review Experience preferred
    • Epic/Health Connect experience preferred
    • Medicare and MediCal benfit knowledge preferred
