We are looking for a compassionate and skilled Case Manager to join our team and make a difference in the lives of our clients.


Job Summary:


Key Responsibilities:


  • Conduct comprehensive assessments to identify clients' needs, strengths, and treatment goals.
  • Develop and implement individualized care plans in collaboration with clients and the treatment team.
  • Monitor and evaluate client progress, adjusting care plans as needed to ensure optimal outcomes.
  • Provide ongoing support and advocacy for clients, assisting them in navigating healthcare systems and accessing community resources.
  • Facilitate group and individual counseling sessions as appropriate.
  • Coordinate with medical, therapeutic, and support staff to ensure holistic care.
  • Maintain accurate and timely documentation of client interactions and progress in accordance with facility policies and regulatory requirements.
  • Assist clients in developing coping strategies and life skills to support their recovery.
  • Educate clients and their families about substance abuse, treatment options, and available support services.
  • Participate in staff meetings, training sessions, and continuing education opportunities to stay current with best practices in substance abuse treatment.
  • Uphold client confidentiality and adhere to ethical standards in all interactions.

Qualifications:


  • CADC Certification or relevant degree
  • Minimum of 2 years of experience in substance abuse treatment or a related field.
  • Knowledge of addiction treatment modalities and community resources.
  • Strong interpersonal and communication skills, with the ability to build rapport with clients and colleagues.
  • Excellent organizational skills and the ability to manage multiple tasks simultaneously.
  • Empathy, patience, and a nonjudgmental attitude towards clients.
  • Proficiency in electronic health records (EMR) systems and KIPU.
  • Ability to work independently and as part of a team.

Pay:
$ $22.00 per hour


Schedule:

  • 8 hour shift
