We are looking for a compassionate and skilled Case Manager to join our team and make a difference in the lives of our clients.
Job Summary:
Key Responsibilities:
- Conduct comprehensive assessments to identify clients' needs, strengths, and treatment goals.
- Develop and implement individualized care plans in collaboration with clients and the treatment team.
- Monitor and evaluate client progress, adjusting care plans as needed to ensure optimal outcomes.
- Provide ongoing support and advocacy for clients, assisting them in navigating healthcare systems and accessing community resources.
- Facilitate group and individual counseling sessions as appropriate.
- Coordinate with medical, therapeutic, and support staff to ensure holistic care.
- Maintain accurate and timely documentation of client interactions and progress in accordance with facility policies and regulatory requirements.
- Assist clients in developing coping strategies and life skills to support their recovery.
- Educate clients and their families about substance abuse, treatment options, and available support services.
- Participate in staff meetings, training sessions, and continuing education opportunities to stay current with best practices in substance abuse treatment.
- Uphold client confidentiality and adhere to ethical standards in all interactions.
Qualifications:
- CADC Certification or relevant degree
- Minimum of 2 years of experience in substance abuse treatment or a related field.
- Knowledge of addiction treatment modalities and community resources.
- Strong interpersonal and communication skills, with the ability to build rapport with clients and colleagues.
- Excellent organizational skills and the ability to manage multiple tasks simultaneously.
- Empathy, patience, and a nonjudgmental attitude towards clients.
- Proficiency in electronic health records (EMR) systems and KIPU.
- Ability to work independently and as part of a team.
Pay:
$ $22.00 per hour
Schedule:
- 8 hour shift
