***: Provides supervision, training and guidance to the nurse case managers/Day Program nurses within the borough. Works with community living management staff to assure necessary supports are in place for the delivery of health care services to persons receiving services.


POSITION DUTIES:


  • Holds regularly scheduled supervisory meetings with the nurse case managers/Day Program nurses to assure delivery of quality health care to persons receiving services.
  • Assists nurse case managers/Day Program nurses as necessary in obtaining health care services for persons receiving services.
  • Oversees the nurse case manager's/Day Program nurses in delegation of nursing tasks to direct care staff and the required associated responsibilities of the nurse, i.e., training.
  • Oversees dissemination of information to concerned parties, including information about acute illnesses, injuries, ER visits, hospitalizations, abuse, elopements, neglect, complex medical problems and a need for informed consent or surrogate decision making.
  • Regularly visits all Residential/Day Program sites within the borough to make certain there is appropriate nursing documentation and follow through. Ascertains all OPWDD regulatory standards regarding medical care needs are being met.
  • Coordinates the health care needs of all persons receiving services in coordination with, borough management, clinics, day programs, hospitals and other service providers. In conjunction with nurse case managers/Day Program nurses and borough management makes sure that all supports are in place.
  • Conducts various trainings including orientation, AMAP, and those geared towards nursing and staff development issues.
  • Attends meetings and participates on committees as needed or assigned.
  • Performs all duties of a nurse case manager for a small caseload and/or when covering in the absence of a nurse case manager.
  • Provides oncall coverage via cell phone after hours and on weekends either as the primary oncall nurse or as a backup for the nurse case manager.
  • Manages nurse case manager caseload assignments and on call responsibilities, as well as nurses' time, attendance, and days off. Prepares evaluations and takes disciplinary action as necessary. Clinical manages Day Program nurses in partnership with Day Program Administrator in preparing evaluations and disciplinary action as necessary.
  • Acts as a resource to others regarding health services and clinical information.
  • Performs miscellaneous duties as assigned or indicated including but not limited to assisting in developing policies, procedures and forms, and developing agency wide systems for the delivery for health care.

REQUIREMENTS:


  • BSN NYS Registered Nurse with five years nursing experience.
  • Supervisory and/or people with developmental disabilities experience preferred.
  • Excellent oral and written communication skills.
  • Ability to travel to and access multiple locations throughout the metropolitan area.
  • Able to work flexible hours and/or days.
  • Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency's corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
