**Currently seeking passionate individuals in all of San Benito County for all positions (i.e. part time, full time, and per diem)


Job Summary

Responsibilities:


  • Conduct comprehensive assessments of patients' health status and develop individualized care plans.
  • Administer medications, perform treatments, and provide wound care as prescribed by physicians.
  • Educate patients and their families on disease management, treatment plans, and selfcare techniques.
  • Monitor patients' progress and adjust care plans as needed to promote optimal health outcomes.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and holistic care.
  • Document patient care accurately and maintain confidentiality of patient information.
  • Provide emotional support and counseling to patients and their families to promote wellbeing and coping strategies.
  • Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.

Home Health Registered Nurse Qualifications:


  • Current Registered Nurse (RN) license in the state of CA.
  • 12 years of clinical nursing experience, preferably in Home Health or Hospice; or a related setting.
  • BLS/CPR certification required; additional certifications such as ACLS or wound care certification preferred.
  • Valid CA Driver's license

Pay:
$90, $130,000.00 per year
