    Status: Full-time

    Mission: To enable frail elderly individuals to age where they prefer and continue leading fulfilling lives.

    The Home Care Coordinator role presents a special opportunity at a new PACE (Program of All-Inclusive Care for the Elderly) program in West Baltimore, under the guidance of a supportive, innovative, and experienced team. Working alongside the Clinic Manager RN, the Home Care Coordinator will play a key role in implementing our care model and making a positive impact on the underserved West Baltimore community.

    • Employ Technology Thoughtfully, Not Aggressively: Utilizing technology to enhance care and connections without replacing human interaction.

    We are seeking an experienced Home Care Coordinator to deliver nursing care to PACE participants and supervise our home healthcare team. This role will report to the Clinic Manager RN.

    Main Responsibilities:
    • Provides skilled nursing care as directed by medical providers to elderly participants.
    • Supervises the home healthcare staff.
    • Coordinates home health service delivery based on participant service plans.
    • Collaborates with the Interdisciplinary Team, participant family members, or caregivers to achieve care plan goals.
    • Communicates with family members or caregivers and offers relevant patient education.
    • Documents test results, observations, and participant conditions in the EMR; Reports reactions to treatments to physicians.
    • Additional responsibilities may be assigned at the manager's discretion due to career growth opportunities and the dynamic start-up environment.

    Qualifications:

    • Graduate of an accredited Nursing Program with Maryland licensure.
    • At least one year of healthcare experience in hospital, long-term, or home health care settings preferred.
    • Experience working with frail or elderly populations for at least one year is desirable.
    • Successful completion of pre-employment screening and background check.
    • Valid Driver's License and compliant driving record; Current automobile insurance required.
    • Updated vaccinations, including COVID vaccine, are mandatory.
