    Location: West Baltimore

    Start Date: June 2024

    Status: Full-time

    Mission: Dedicated to enabling elderly individuals to age in their preferred surroundings and maintain meaningful, connected lives

    The Home Care Coordinator position presents a unique opportunity within a new PACE (Program of All-Inclusive Care for the Elderly) program in West Baltimore. You will work alongside a supportive, innovative, and expert team under the leadership of our Clinic Manager RN. Together with our coalition of community partners, you will have a significant impact on an underserved population in West Baltimore.

    We are seeking an experienced Home Care Coordinator to provide nursing care to PACE participants and oversee our home healthcare team. Reporting to the Clinic Manager RN, the main responsibilities include:

    • Providing skilled nursing care to elderly participants as directed
    • Supervising the home healthcare staff
    • Scheduling home health services based on participant service plans
    • Collaborating with the Interdisciplinary Team, family members, and caregivers to achieve care plan goals
    • Communicating with family/caregivers and offering patient education
    • Recording participant information accurately and reporting their condition to physicians
    • Adapting to additional responsibilities as needed in our dynamic environment

    Qualifications:

    • Graduate of an accredited Nursing Program with a Maryland license
    • Minimum of one year of healthcare experience, preferably in a hospital or home health setting
    • Prior experience with elderly or frail populations is a plus
    • Successful completion of pre-employment screenings and background checks
    • Valid Driver's License and compliant driving record
    • Current vaccinations, including COVID vaccine, are required
