- They will initiate the development of the treatment plans based upon the strengths and needs identified in the family IFSP and CANS.
- Provide support and consultation of the STARS resource parents, to the families of children in care and to other team members related to their role in the treatment plan.
- Coordinates and links for children and their families with needed services available within the agency or greater community in collaboration with the assigned Advocate.
- One to two years of related experience with working with children, adolescents, and families providing direct professional interventions within the scope of their practice and limits of their expertise.
- It is required of all professional staff, including support service workers, supervisor, social worker, and other licensed staff, to have pre-service and ongoing professional development relevant to the treatment resource home care model and their individual job responsibilities.
- Experience in community work and knowledge of community resources.
- Position requires reliable transportation, valid driver's license, and current automobile insurance coverage.
