POSITION SUMMARY
Under the direction of the Community Health Outreach Manager, the Community Health Representative (CHR) assists clinic clients and/or Tribal members with general medical and social service needs, including patient advocacy, care coordination and accompanying to medical appointments.
ESSENTIAL FUNCTIONS
The Nottawaseppi Huron Band of the Potawatomi reserves the right to change, amend, add, delete and otherwise assign any and all duties, responsibilities, and positions titles as it deems necessary to meet the needs of the government.
Assists with home monitoring of clients and providing direct patient care within scope and capacity as identified in the client care plan, i.e., basic screenings, and basic needs as directed by the Provider and or Clinic Care Manager - RN.
Assists with pickup and delivery of pharmacy orders and or other medical related items per policy.
Provides education on health services and programs offered by CHR and HHSD.
Motivates and encourages clients to obtain care and other services by facilitating community-clinic linkages through internal and external resource coordination and referral.
Participates in monthly case conference meetings and gives feedback and updates as necessary.
