- Good knowledge of community resources.
- Completion of initial training program and ongoing educational requirements.
- Technological proficiency with basic computer skills.
- Prior Home Care experience is a plus.

About the Role:
The Care Specialist at Upward Health primarily operates in patients' homes and communities, dedicating 90% of the time to delivering chronic care management to high-complexity patients. Additionally, the role involves virtual interactions for the remaining 10% of the time.
Responsibilities:
Key Responsibilities:
- Conduct direct outreach to patients via phone, in-person, and mailings
- Enroll patients into the program and collect key data about patients during enrollment
- Meet patients in their home or community to conduct a needs assessment and help set health goals
- Coordinate care between patients and interdisciplinary care teams
- Support patients in meeting their healthcare goals related to Quality measures
- Analyze patient data to determine patient needs or treatment goals
- Explain technical medical information to educate patients
- Cultivate and support primary care providers with timely communication and integration of information into care plans
- Build rapport with patients using motivational interviewing techniques
- Conduct in-person patient appointments and home visits
- Document each patient encounter accurately
- Prepare reports and documents as needed
- Attend regular team meetings and participate in clinical rounds
- Other duties as assigned
