JOB SUMMARY:


PRINCIPAL DUTIES AND RESPONSIBILITIES:


  • Responsible for managing previsit planning activities for assigned primary care and OB/GYN providers. Planning activities include but are not limited to lab and imaging tracking.
  • Facilitates quality program education and training with providers and staff to ensure clear understanding of measure definitions, work flows, and data capture. Assesses challenges related to the electronic health record (EHR) and work flows and provides recommendations for improved performance.
  • Manages daytoday activities for payor quality programs including Aetna on Track, Anthem Inovalon, and Optum Care Manager.
  • Supports the monitoring of MIPS performance and assists in the preparation of the annual data submission to the Centers for Medicare and Medicaid Services (CMS).
  • Prospectively and retrospectively works to identify and close gaps in care through communication with providers, practices, patients and community resources.
  • Participates in data validation for EHR reports as required.
  • Develops strong relationships with providers, practice managers, and medical assistants.
  • Attends and participates in quality team meetings.
  • Attends scheduled staff meetings, in service education opportunities, and participates in professional development opportunities.
  • Formulates and maintains effective working relationships with employees and other managers of the health care team.

QUALIFICATIONS/REQUIREMENTS:


  • Bachelor's degree required.
  • Two or more years of related professional experience in a medical group setting is strongly preferred.
  • Experience with electronic health records (EHRs) is required. Experience with eClinicalWorks is preferred.
  • Working knowledge of population health programs including Merit-Based Incentive Payment System (MIPS), Patient-Centered Medical Home (PCMH) and Healthcare Effectiveness Data and Information Set (HEDIS) is preferred.
  • Proficient in

Microsoft Office:
Excel, Word, and PowerPoint.

  • Strong written and verbal communication skills.
  • Superior organizational and interpersonal skills.
  • Detail and process orientated with a focus on efficiency.
  • Ability to work on a team.
