- Performs comprehensive history and physical on new referrals. Performs interval history and physical on established participants done quarterly or as indicated.
- Manages patients with acute, episodic, and chronic illnesses.
- Assumes ongoing responsibility for each patient under her/his care.
- Refers participants to medical director or medical specialist as indicated
- Admits participants to the hospital, provides primary care in conjunction with Medical Director, provides updates to multidisciplinary team on a regular basis, judiciously utilizes special consultants and coordinates discharge planning with team.
- Manages care of participants in the nursing home through regular visits, telephone contact with nursing home staff and admits nursing home participants to hospital when necessary.
- Actively participates in family conferences. Attends in interdisciplinary team meetings and contributes to the interdisciplinary team.
- Participate in committees and work groups as assigned.
- Documents treatment in patient charts according to established department guideline.
- Graduate from an accredited school of nursing
- Advanced training and certification as an Advanced Registered Nurse Practitioner (ARNP) or advanced training and licensure as a Physician Assistant (PA).
- 2-3 years' experience preferred; minimum of 1 year specifically working with frail elderly population.
- Current ARNP/PA license by the Florida State Board of Nursing
- BLS certification required
- ACLS certification preferred
- Drug Enforcement Administration (DEA) License required
- Knowledge of nursing care practices for rehabilitation, geriatric and dementia patient populations preferred.
- Demonstrates a caring attitude toward residents, patients, family members, employees, and other facility guests
- Normal working condition with adequate lighting and ventilation
- Exposure to toxic or caustic chemicals; bio-hazardous waste; flammable, explosive gas
- Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from pounds.
- Routine exposure to blood/body fluids/tissues
- Ensures that resident's/patient's rights are adhered to
