    Job Summary:


    Interim HealthCare, an industry-leading home healthcare and medical staffing agency, is seeking a skilled and compassionate Registered Nurse (RN) to join our team as Case Manager in Charlottesville, VA.

    This is a contracted position with a pay rate of $48 per hour with day shift availability
    You must have at least 2 years of recent experience.



    Compensation and Benefits:

    • Competitive hourly rate of $44 per hour
    • Paid weekly
    • Flexible schedule

    Responsibilities:

    • Develop, coordinate, and monitor clientspecific healthcare plans
    • Conduct initial and ongoing assessments of each client's healthcare needs
    • Communicate with physicians, caregivers, and other healthcare professionals to ensure coordinated care
    • Maintain detailed and accurate documentation of client care and progress
    • Provide skilled nursing care, including wound care, medication administration, and other treatments
    • Foster a compassionate and supportive relationship with clients and their families
    • Advocate for clients' rights and promote selfadvocacy
    • Maintain compliance with all federal, state, and local regulations
    • Participate in continuous education and professional development opportunities

    Requirements:

    • Active and unrestricted Registered Nurse (RN) license in the state of Virginia
    • Experience in case management is preferred
    • Excellent communication and interpersonal skills
    • Strong organizational and timemanagement abilities
    • Ability to work independently and as part of a team
    • Must pass a comprehensive background check and drug screening
