  • The palliative RN Navigator is responsible for identifying and coordinating patient/family care to support the palliative team, patients, and families as they care planning.
  • In conjunction with the palliative team, endeavors to utilize teaching, assessment, and intervention skills to provide symptom management and maximize the quality of life for the patients and families.
  • Maintains ongoing effective communicaon with other palliative care team personnel and others who are involved with patient care.
  • At times, act as a triage RN Navigator for the palliative care team.
  • Assist patients, family members, or other clients with concern and empathy; respect confidentially and privacy and communicate courteously and respectfully.
  • Assesses home care and hospice needs, being aware of the physical, emotional, and spiritual aspects, and gathers data on social, economic, and cultural factors that may influence health, wellbeing, and quality of life.
  • Attend and participate in weekly IDT meetings.
  • Coordinates community resources and other agency disciplines participating in patient care.


The RN Navigator works with a multidisciplinary team to plan and coordinate the delivery of care for a disease specific patient population across the Medical Center.


The RN Navigator provides education and support to patients and their families facilitates transitions of care to assure that patients have appropriate referrals and follow-up appointments post discharge and works with the multidisciplinary team to implement and sustain evidence based practice.


The main objectives of this role are to facilitate patient self-management prevent readmission increase patient satisfaction and ensure that patients receive necessary services.

The RN Navigator will work closely with other disciplines to achieve quality patient care.

Collaborates with care management to evaluate and follow patients upon admission for potential needs medication assistance and community resources.
Plans individualized care in collaboration with the. Multidisciplinary team.
Gathers information necessary to guide the development of the self-management plan for the patient. This information may include psychological assessment literacy levelsocial assessment and economic barriers.
Identifies and coordinates the care of disease specific population specific inpatients.
Provides reinforces and documents patient and/or caregiver disease specific and health maintenance education.
Identifies potential barriers to effective disease management and/or etiology for readmissions.
Works with established multidisciplinary teams to develop appropriate interventions to address barriers.
Implements a plan to address short and long term care needs.
Manages and coordinates pre and post-discharge care to include:
Assists with evaluation of patient care outcomes.
Demonstrates leadership in the delivery of patient care at the Clinician 3 level.
Coordinate patient care and education.
Assure continuity of care.
Participates in performance measurement and improvement activities.

Position Compensation Range:
$ $54.52 Hourly


MINIMUM REQUIREMENTS

Education:
Graduate of an accredited nursing program. Bachelor's of Science in Nursing Degree preferred.

Experience: 1 year of relevant experience.

Licensure:
Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. American Heart Association (AHA) Health Care Provider BLS certification required. May require a valid Virginia Driver's License (VADL).


PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling and bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
