Job Summary:


DES, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a District Nurse.

DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven.

These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.


Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.


This position may offer the ability to work remotely, within Arizona, based upon the department's business needs and continual meeting of expected performance measures.

Job Duties:
Essential Duties and Responsibilities include, but are not limited to:

  • Assesses physical and behavioral health needs and completes nursing assessment of individuals with developmental disabilities based on need; makes nursing diagnosis and submits to designated departments and documents same. Addresses other medical needs as requested by Health Care Services and/or District.
  • Completes additional documentation pertaining to care coordination of physical and behavioral health needs of division members. Attends staffing and discharge planning meetings and talks/meets with appropriate health care professionals to network to provide continuity of care for members.
  • Completes utilization review on caseload, documents ongoing medical needs, and submits information to designated department
  • Attends ISP and IEP meetings as the medical/health component to provide medical information and needs of member as they relate to the ISP or IEP; Attends statewide nursing meetings, monthly nursing meetings, quarterly home health agency meetings, case conferences on caseload, inservice workshops, and seminars
  • Assesses nutritional status and needs of member and makes referral to RBHA, Dietician, Primary Care Physician, or health plan as appropriate; Assists in assigned areas to complete audits/fact finding reviews; Monitors cost effective studies on assigned members

Knowledge, Skills & Abilities (KSAs):

Knowledge in:

  • General nursing theories and practices specifically related to nursing care for individuals with developmental disabilities and behavioral health issues
  • Nutritional needs
  • Interdisciplinary team planning process
  • Services offered in community including primary health care providers
  • State Nurse Practice Act
  • Title XIX federal regulations and licensing standards

Skills In:

  • Observing and assessing health status and needs of individuals with developmental disabilities
  • Providing appropriate treatment in emergency situations
  • Oral and Written communication, including effectively communicating technical information to members and families
  • Computer skills
  • Developing, analyzing, and implementing treatment care plans
  • Effective problem solving

Ability to:

  • Listen, observe, assess, and respond to medical emergencies
  • Evaluate, analyze, make independent decisions, and follow up regarding member health problems
  • Present training material in a clear and precise manner
  • Respond to medical issues/inquiries in a professional manner
  • Work with professional and nonprofessional staff to ensure health and safety of residents

Selective Preference(s):

  • At least 1 year experience providing professional handson nursing services to individuals with developmental disabilities and at least 1 year of experience in professional community nursing.

Pre-Employment Requirements:

  • Required at time of appointment: possession of a current license to practice as a registered nurse in the State of Arizona.
