Position Summary:

New York Connects is a point of entry into long term services and support systems for older adults and people of all ages with disabilities.

NY Connects is a program designed to provide intensive information and referral service to address the needs of individuals in the borough of Queens.

Selfhelp Community Services is seeking an Outreach Coordinator for the New York Connects Program in Flushing, Queens. The Outreach Coordinator will provide outreach and education activities to increase awareness of the program.


Responsibilities:


  • The Outreach Coordinator will assist the Program Director with program outreach and promotion of New York Connects to the community.
  • The New York Connects Outreach Coordinator will provide information (face to face) about the program to the community at weekend events, fairs, libraries, OAC's.
  • Screen individuals inperson to determine needed services.
  • Determine eligibility for government entitlements and benefits based on financial screening.
  • Provide resource information, and linkages to long term care services and supports; where necessary, the Outreach Coordinator will obtain contact information for the purpose of followup work with a New York Connects Specialist.

Salary Ranges:
$45 - $50 per hour commensurate with experience.


Qualifications:

MSW required
:


  • Bilingual a plus.
  • Strong verbal and written communication skills, including documentation.\
  • A minimum of three years' experience in the social service field working with either an older adult population, persons with developmental disabilities, persons with mental health needs, or a related area of human services.
Compassionate, culturally sensitive with excellent listening skills.
