Full Time:

PermanentEmployedBE/BC in FM/IM and Geriatric Fellowship highly desirableClinical/outpatient services for high-risk older adults in subacute and long-term skilled nursing facilitiesThe physician will join a primary care team consisting of physicians and advanced practice providers who specialize in this careThe full-time positions are located in the primary care clinics on our Austin and Albert Lea CampusesThe physician will work collaboratively with advanced practice providers to provide holistic care including acute, chronic, preventive and palliative care servicesThe provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments and non-pharmacologic therapies, and counseling patients including advanced care planningIdeal candidates have experience in Primary care, Geriatrics and Palliative CareTravel will be required throughout the Southeast Minnesota region and may include shared time in RochesterThe physician working in the facilities may have the opportunity to be the medical director of the facility as wellAccess to collaborative consults from health system specialty physicians, in addition to onsite pharmacy and social workGood balance of subacute and long-term care patients, with medical directorship opportunities availableOpportunity for a blended practice of primary outpatient internal medicine or family medicine, and senior services care deliveryMentorship and coaching support availableOpportunity for education and researchCommunity: Austin, MN is a community of 22,000, and is surrounded by prairies, rivers and valleys just 90 minutes south of Minneapolis/St.
